Position Overview

The Healthcare Performance Improvement Manager is responsible for cultivating reliable relationships with healthcare providers and their clinical teams. This role involves collaborating with leadership and support staff within the assigned provider practices in the Equality Health Network to implement the Equality Health Care Model. The primary objective of this position is to transform practice performance from a reactive patient care model to a proactive accountable care approach, utilizing population data to effectively measure, monitor, and manage patient care while presenting results to executive leadership.

Key Responsibilities

The Healthcare Performance Improvement Manager will:

  1. Enhance care for high-risk, complex patients by supporting practices in utilizing the CareEmpower Chart Prep Tool for effective management and ensuring regular visits to their primary care provider (PCP).
  2. Minimize avoidable admissions and readmissions by assisting practices in tracking and managing care transitions for all inpatient discharges, ensuring follow-up with PCPs and specialists within a week.
  3. Boost preventive wellness visit rates by collaborating with practice quality teams to establish weekly targets for adult and child wellness visits, identifying and addressing barriers to patient engagement.
  4. Implement Care Empower, the provider technology platform that identifies care gaps.
  5. Facilitate the integration of Equality Health's digital data retrieval systems with compatible practice electronic medical records (EMRs).
  6. Work closely with assigned practices to assess current workflows and implement improved care processes for high-risk patients, ensuring timely follow-up and management.
  7. Establish regular meetings with assigned practices to monitor progress on practice transformation and performance goals related to wellness, care transitions, and high-risk patient management.
  8. Collaborate with various teams within the Equality Health network to support practices effectively.
  9. Provide monthly performance summary reports to practices and utilize data analysis to highlight areas for improvement and necessary changes.
  10. Maintain accurate documentation of provider and practice demographics in the client relationship management platform.
  11. Conduct training sessions on Cultural Care, HEDIS, STARS, correct coding, and RAF as required.

Qualifications

  1. Bachelor's degree in business, healthcare administration, or a related field; or equivalent experience in healthcare delivery systems.
  2. A minimum of three years of experience in the healthcare sector, preferably within provider practices.
  3. Strong verbal, written, and interpersonal communication skills.
  4. A collaborative approach to teamwork.
  5. Experience or exposure to change management.
  6. Familiarity with electronic health records.
  7. Proficiency in Microsoft Office and web-based technologies.
  8. Willingness to travel to assigned practices as needed.

Preferred Skills and Qualifications

  1. Understanding of patient and practice risk adjustment mechanics.
  2. Proven ability to engage providers and support staff to achieve mutual care management goals.
  3. Critical thinking skills to identify problems and develop effective strategies for resolution.
