Key Responsibilities:- Work effectively with the Integrated Care Team to provide comprehensive care management to members.
- Utilize brief medical interventions to improve members' ability to manage their own health.
- Provide formal and informal training and support for team members on medical conditions, including treatments and evidence-based practices.
- Assist members with care coordination needs, including conducting comprehensive assessments, developing individualized care plans, and communicating with members and care teams.
- Coordinate with internal and external health partners to support members' comprehensive care needs.
- Model the highest ethical behavior in relationships with coworkers, supervisors, members, providers, and colleagues in the community.
- Engage in evidenced-based communication strategies to promote collaboration and effective working relationships.
- Minimum two years of clinical experience in an acute care facility, skilled nursing facility, home health, or clinic setting.
- Active, unrestricted, and unencumbered Registered Nurse (RN) license issued by the California BRN.
- Two or more years of care management experience in a healthcare delivery setting preferred.
- Bilingual (English/Spanish) preferred.
- Highly skilled interpersonally, with excellent relationship skills.
