Job Summary

We are seeking a highly skilled Care Manager, RN to join our team at HealthCare Talent. As a key member of our Integrated Care Team, you will be responsible for providing high-quality care management to our members.

Main Responsibilities
  • Work effectively with the Integrated Care Team to provide comprehensive care management to members.
  • Conduct comprehensive, holistic assessments of members' medical needs and develop individualized care plans.
  • Coordinate with internal and external health partners to support members' comprehensive care needs.
  • Lead interdisciplinary care team meetings to share information and update care plans.
  • Model the highest ethical behavior in relationships with coworkers, supervisors, members, providers, and colleagues in the community.
  • Build and maintain positive working relationships with providers, including communication via in-person, phone, and digital means.
  • Engage with members in a manner that utilizes evidence-based approaches to promote collaboration and increase members' sense of control over their whole health.
  • Model commitment to continuous quality improvement by engaging in quality improvement initiatives and projects.
Requirements
  • Minimum two (2) years clinical experience in an acute care facility, skilled nursing facility, home health, or clinic setting.
  • Possession of an Active, unrestricted, and unencumbered Registered Nurse (RN) license issued by the California BRN.
  • Two (2) or more years of care management experience in a healthcare delivery setting preferred.
  • Experience in a Managed Care (HMO, IPA) or in acute facility (i.e. hospital) care management preferred.
Preferred Qualifications
  • Bilingual (English/Spanish) preferred.
  • Highly skilled interpersonally, with excellent relationship skills.
  • Highly skilled in interpersonal communication, including resolving conflict with coworkers.
