Position Overview

The Practice Performance Manager (PPM) plays a crucial role in building reliable relationships with healthcare providers and their support teams. This position is focused on collaborating with leadership and staff within assigned practices in the Equality Health Network to implement the Equality Health Care Model. The primary objective of the PPM is to transition practice performance from a reactive patient care model to a proactive accountable care framework. This involves analyzing population data to effectively measure, monitor, and manage patient care, while presenting outcomes to executive leadership.

Key Responsibilities

The Practice Performance Manager (PPM) will support clinical teams in the following critical areas:

  1. Enhancing care for high-risk, complex patients by utilizing the CareEmpower Chart Prep Tool to ensure consistent management and quarterly visits with their primary care provider (PCP).
  2. Minimizing avoidable admissions and readmissions by effectively using CareEmpower to track inpatient discharges and ensure timely follow-ups with PCPs and specialists.
  3. Boosting preventive wellness visit rates through collaboration with practice quality teams to set expectations and address barriers to patient engagement.
  4. Implementing Care Empower, a technology platform designed to notify providers of care gaps.
  5. Facilitating the integration of Equality Health's digital data retrieval systems with compatible electronic medical records (EMRs).
  6. Driving workflow improvements in practices to ensure high-risk patients receive appropriate care and follow-up.
  7. Establishing regular operational meetings with practices to monitor progress on performance and transformation goals.
  8. Collaborating with various teams within the Equality Health network to enhance practice support.
  9. Delivering monthly performance reports to practices and utilizing data analysis to identify opportunities for improvement.
  10. Maintaining accurate documentation of provider and practice demographics in the client relationship management platform.
  11. Conducting necessary training sessions on cultural care, HEDIS, STARS, coding, and RAF.

Qualifications

  1. Bachelor's degree in business, healthcare administration, or a related field; or equivalent experience in healthcare delivery systems.
  2. At least three years of experience in the healthcare sector, preferably within provider practices.
  3. Strong verbal, written, and interpersonal communication skills.
  4. A collaborative approach to teamwork.
  5. Experience or exposure to change management.
  6. Familiarity with electronic health records.
  7. Proficiency in Microsoft Office and web-based technologies.
  8. Willingness to travel to assigned practices as needed.

Preferred Skills and Qualifications

  1. Understanding of patient and practice risk adjustment mechanisms.
  2. Proven ability to engage providers and drive performance improvements.
  3. Critical thinking skills to identify and resolve issues effectively.
