Position Overview

Project Coordinator, OHT and Transformation
We are seeking a skilled Project Coordinator to facilitate transformative healthcare initiatives within the North York Toronto Health Partners (NYTHP). This role involves collaborating with various stakeholders to enhance healthcare delivery across North York and beyond.

The successful candidate will engage in the execution of several key initiatives, including:
- Enhancing care transitions for youth facing mental health and addiction challenges;
- Implementing the North York Congregate Access and Support Team (NYCAST) to improve health outcomes for individuals in long-term care and congregate settings;
- Supporting the Integrated ALC Committee to promote aging in place;
- Advancing OHT Acceleration efforts related to governance models and information-sharing among partners.

In this role, you will collaborate with NYTHP members and contribute to strategic priorities aimed at promoting health equity and system integration. As we redesign healthcare delivery, your innovative approach will be essential in addressing the evolving needs of our initiatives and the community we serve.

Key Responsibilities


• Oversee the planning and execution of OHT and NYGH strategic initiatives throughout various phases.

• Organize and facilitate project meetings, prepare reports, and maintain comprehensive project documentation.

• Cultivate and sustain relationships with project partners, ensuring effective communication.

• Identify potential project risks and propose solutions and mitigation strategies.

• Ensure timely delivery of project outcomes within budget and scope.

• Provide administrative support, including agenda setting, minute-taking, and tracking action items.

• Monitor metrics for select OHT initiatives, including Collaborative Quality Improvement (c-QIP) indicators.

• Analyze evaluation and public health data to inform program enhancements.

• Assist in planning and facilitating cross-organizational meetings and events.

• Conduct research to support OHT Committees and strategic goals.

• Develop content for periodic reporting to relevant health authorities.

• Aid in preparing grant applications and exploring funding opportunities.

• Serve as a key participant in the Integrated ALC Committee.

• Actively engage as a member of the OHT Backbone team, attending meetings and providing updates.

• Perform additional duties as required.

Qualifications


• An undergraduate degree in Health Administration, Health Sciences, or a related field, along with a minimum of four years of experience in health system or policy project support.

• Familiarity with public health and health policy evaluation methodologies.

• Project Management training or certification is an asset.

• Exceptional verbal and written communication skills.

• Strong organizational and time-management abilities.

• Proficient in conveying data insights to internal and external stakeholders.

• Excellent relationship management and community engagement skills.
