About the Role

VHA Home HealthCare is a leading provider of home care services in Ontario, with a team of over 3,000 dedicated professionals. As a Professional Practice Leader, you will play a key role in advancing clinical practice, research, education, and interdisciplinary models of care in the fast-changing home healthcare landscape.

Key Responsibilities
  • Provide clinical leadership and expertise to advance professional practice, research, education, and interdisciplinary models of care.
  • Implement Evidence-Based Best Practices to enhance care outcomes, promote healthy work environments, and ensure excellence in care and service delivery.
  • Ensure that skills, competencies, and enhanced practices are in accordance with regulatory bodies, VHA policies, and procedures.
  • Contribute to the design of new programs and practice initiatives, providing clinical expertise and determining healthcare team members' informational needs.
  • Provide staff education and learning supports, and prioritize key clinical practice areas to maximize clinical safety and care effectiveness.
Requirements
  • Registration as a Registered Nurse (RN) in good standing with the College of Nurses of Ontario, RNAO member, with preference given to candidates with CCHN(C) certification.
  • Masters' degree in a Health-related field required (candidates who can demonstrate they are actively pursuing their Masters degree will be considered).
  • Successful candidate will be a strong clinical practitioner with an ability to use knowledge-transfer techniques to put research–based knowledge into practice.
  • Minimum 2 years Acute Care experience and familiarity with wound/post-surgical outcome-based pathways required.
  • Understanding of health care service delivery in community sector considered an additional asset.
  • Nurse specialized in wound, ostomy and continence (NSWOC) certification an asset.
  • Minimum 1 year of project management and leadership required.
  • Demonstrated expertise in facilitation, sustainability, and evaluation of program outcomes.
  • Knowledge of adult education principles and a demonstrated ability for provide leadership in education to develop, deliver, or facilitate education programs for staff and service providers.
  • Understanding of learning needs for both interdisciplinary regulated and unregulated healthcare providers.
  • Experience in educational program development and clinical teaching an asset.
  • Highly motivated, with demonstrated ability for self-directed, independent work and as a collaborative team member.
  • Organization and self-management skills.
  • Superior interpersonal, verbal, and written communication skills.
  • Demonstrated ability to problem-solve effectively.
  • Desire to champion interprofessional practice and collaboration.
  • Demonstrated commitment to a Person & Family Centred Care model of care and service delivery.
  • Evidence of ongoing professional development and a demonstrated commitment to continuous learning.
  • Proven track record of achieving personal and organizational performance targets.
