**Position Description**:
The SCAN program is currently seeking a Program Coordinator to provide clinical, coordinator and case management support to the patients, families, and team members of the SCAN program. This position will report to the Sr Clinical Manager.

**Here¿s What You¿ll Get To Do**
- Manages the referral and scheduling process from intake through to coordinated appointment bookings. This includes performing intake activities with the patients and families prior to their visit.
- Performs duties of Patient Flow Coordinator to include patient registration, obtaining anthropometrics, managing clinic flow and organizing follow up appointments and tests.
- Obtains medical records and additional information from external resources.
- Supports and coordinates retrieval of patient prescriptions and medical supplies as necessary.
- Manages patient health records.
- Maintains statistical data for the sexual assault program and sends report quarterly to the Sexual Assault Provincial Network.
- Collecting and compiling data, sending to appropriate stakeholders, analyzing when necessary.
- Manages patient amenities fund.
- Leads daily SCAN status meetings.
- Plan, coordinate and support parent education sessions in collaboration with interdisciplinary team.
- Manages call schedule.
- Manages inventory of supplies for clinic and for program.
- Point of contact for patients, families, vendors, other hospitals, Children's Aid Society, CYAC, Toronto Police.
- Coordinates complex case conferences with external agencies.
- Manages and tracks full cycle of Peer Review Process.
- Manages and tracks full cycle of Quality Assurance Process.

**Here¿s What You¿ll Need**
- Undergraduate degree or the equivalent of education/experience with at least 2 years of administrative experience, preferably working in a health care environment.
- Good judgment, communication, interpersonal and critical thinking skills.
- Efficient time management and strong organization skills to manage multiple priorities with mínimal direction.
- Demonstrated motivation and ability to anticipate, analyze and take initiative.
- Demonstrated ability to provide customer service to patients and families that is sensitive to their diversity and complexity.
- The Program Coordinator must also maintain, share and organize complex, confidential, sensitive information when coordinating appointments and/or meetings.
- A commitment to understanding and aiding in the pursuit of equity, diversity & inclusion.

**Employment Type
- **Temporary Full Time-0.90FTE. 5-month contract
