Acute Stroke Coordinator

We currently have a** Regular Full-time **opportunity for a **Acute Stroke Coordinator** on the **Regional Stroke **Department at our **Bayview** campus.
**Hours of Work**: Weekdays: & Alternating Weekends**:Days, Evenings, and Nights; 12hrs; As per schedule**, Statutory Holidays and On Call Required**
Summary of Duties
Reporting to the Director of the Regional Stroke Centre, the Acute Stroke Coordinator will enhance stroke care by coordinating logistics of care to increase efficiency of care processes, communicating with the interprofessional care team, liaising with patients/family members regarding clinical status, education, and repatriation, conducting data collection, and participating in quality improvement initiatives.
**Tasks**
- Collaborates in the development, implementation and evaluation of an organized stroke care delivery system
- Collaborates with the interprofessional team to enhance the provision of stroke care that incorporates clinical standards best practices, and pathways, and enhances care/service delivery outcomes
- Performs independent/interdependent advanced stroke assessments in support of the Stroke Team as per Stroke Best Practice Guidelines
- Coordinates hyperacute stroke care (provision of EVT and tPA) by liaising with other services and team members as required to facilitate interprofessional care delivery and efficient movement of the patient
- Facilitates the flow of stroke patients throughout their care experience. This includes coordination with all teams involved in stroke care
- Liaises with patients and family members to facilitate care coordination, and to enhance the patient and family experience
- Participates in delivery of educational initiatives to meet patient, family and healthcare professional learning needs
- Maintains constant readiness for system-wide stroke care
- Keeps informed of current best practice guidelines and research evidence
- Collect key data elements and participate in quality improvement and process monitoring to support continued implementation of best practice stroke care
Qualifications/Skills
- Regulated health professional (Occupational Therapist, Physiotherapist, Registered Nurse, Speech Language Pathologist)
- Registration and good standing with Professional Regulatory College
- Minimum 5 years’ experience with neuro / stroke care required
- Current Basic Cardiac Life Support certificate required
- Demonstrated knowledge of relevant clinical/stroke related best practices within the management of Complex Neuro / Stroke cases that bridge the full continuum of care
- Recent related direct care experience with acute stroke care and rehabilitation / recovery and complex cases
- Demonstrated interprofessional team leadership skills and superior interpersonal and communication skills
- Demonstrated ability and insight to determine boundaries in scope of practice and role limitations
- Ability to work effectively both autonomously and within an interprofessional team environment
- Ability to problem solve, prioritize and manage time effectively in a fast-paced environment
- Demonstrated understanding of person-centred care and the ability to utilize the principles in daily practice
- Experience with patient and family education
- Experience with data collection and in utilizing data to collaborate with the team to support quality improvement
- Computer skills (MS Word, PowerPoint, Excel, database software), or willingness to develop skills
