**Pilgrims Hospice Society (PHS) is a registered charity that provides supportive and compassionate family-centered care to enhance the quality and dignity of life for those diagnosed with a progressive, life-limiting illness as well as support to those who are grieving the death of a loved one, from any cause.**

**POSITION DESCRIPTION - **GRIEF SUPPORT PROGRAM INTAKE COORDINATOR/PROGRAM ASSISTANT**

Utilizing their educational background, the Grief Support Intake Coordinator/Program Assistant provides administrative support, including registering participants, program planning with the team, set up of session, and clean up. This position will require the individual to process the intake of our new clients and set up initial appointments for our counselling team. Clients vary as we provide a wide range of psycho-educational services to our Counselling clients, Day Program clients, as well as the terminally ill residents and their families in our Residential Hospice Program. Clients range from young children to seniors. The death causes are varied, including natural causes, illness, accident, suicide, murder, child loss, baby loss and opioid overdoses.

**ATTRIBUTES/QUALIFICATIONS**

The Grief Support Intake Coordinator/Program Assistant must be knowledgeable of/have the ability to:

- Use discretion and judgement to support counselling clients, residents and their families with the complex emotional needs associated with terminal illness and end of life, anticipatory grief, and bereavement by analyzing their emergent needs to determine appropriate placement within our counselling practice and group programming.
- Understand family dynamics, unique needs and patterns/styles for communication and/or decision-making to support the grief journey if appropriate in the initial intake and within the group settings.

**KEY RESPONSIBILITIES**
- Abides by and demonstrates PHS’s Mission, Vision and Values through both behavior and job performance on a daily basis.
- Conducts an initial assessment of the client/resident that focuses on the dynamic needs of the client/resident and family.
- Sets up initial client records. Inputs Stats for team members into our Common Drive for review and use by other staff members.
- Books rooms for counselling, groups, presentations and workshops provided by the Grief Counselling Team.
- Attends and participates actively in Interdisciplinary Team Meetings, staff meetings, educational in-services, and actively contributes to quality outcome measures.
- Maintains our in-house Lending Library and orders and catalogues new materials.
- Ensures program supplies are up to date and ordered ahead of groups, presentations and workshops.
- Researches varied mediums for current data, quotes and potential literature for handouts. These are distributed to counselling clients, as well as group, presentation and workshop attendees.
- Ensure filing of documents and files is kept up to date in a timely manner.
- Reviews and adheres to all company policies and procedures as outlined in the Employee Handbook.
- Participates in special projects and performs other duties as required.
- Hours will generally be Monday to Friday - 8:30 am to 4:30 pm. Hours may vary due to programming, events and/or special projects and may include some evenings and weekends.

**REQUIREMENTS**
- Current registration with appropriate professional association if applicable.
- Prior non-profit experience an asset.
