Intake Specialist
Purpose
The Intake Specialist position supports all areas of program delivery by working with families, individuals, service providers and other stakeholders to determine intake eligibility and funding requirements. This position is responsible for recommending appropriate support services within the organization according to; individual support requirements, program criteria and funding resources. The conscientious Intake Specialist will act with integrity, be open to growth, live our purpose, cultivate teamwork, and put forth an extraordinary effort to empower people with special needs and their families to grow, succeed, and belong.
Key Functions
- Complete a needs assessment/level of support for new and returning families within AdaptAbilities.
- Confirm or advocate for appropriate funding based on need
- Conduct initial intake meetings with individuals and parents to determine individual needs.
- Provide support and resources to families
- Ensure all required information for participants is received and processed in a timely manner
- Assist individuals and their families to effectively navigate internal and external processes
- Perform a hazard assessment for all potential In-Home participants by visiting the home
**Qualifications**:
- Diploma or degree in the human services field preferred, or an equivalent of experience 2+ years of experience in the human services field
- 1 years’ experience providing short-term case management, facilitating collaborative and individualized service implementation.
- Experience with Microsoft Office 365
- Clear police information check and intervention record check
- First Aid Certification with CPR, Non-Violent Crisis Intervention, Abuse Prevention and Response Protocol
- Maintain a driver’s license
Working Conditions
This position is a permanent, full-time position, Monday to Friday 8:00 am to 4:30pm reporting to the Program Manager.

**Compensation Package**: $48,750 - $51,750
