Referral and Intake Coordinator

The Referral & Intake Coordinator is one of the first points of contact for adults with a brain injury who live in community and require community resource navigation. The role is responsible for providing support and resources, as well as screening for eligibility and directing ineligible individuals to appropriate services. This position will also provide case management to a small caseload of clients to assist them with identifying goals and obtaining the clinical and community support they need to reintegrate and regain independence.
**Qualifications**:
Certification: Certificate or diploma is human services, medical administration, or a related field.
Experience: 1-2 years of experience with medical office referral processes, or human services programming.
Must provide an up-to-date and clear criminal record check.
Access to a vehicle to use for daily commute to work with $2 million liability insurance.
**Responsibilities**:
Collect new referral information and any relevant medical documentation for a variety of programs of services, related to types of brain injury, concussion, or caregivers of those with brain injury.
Obtain and screen all new referrals for eligibility, and investigate any potential concerns (i.e. history of aggressive behaviour) and recommendations to mediate potential concerns.
Ensure that all client referrals are responded to and completed in a timely manner.
Liaise with community rehabilitation partners to determine future needs of clients prior to discharge from hospital.
Provide community resources and necessary support to individuals with disabilities.
Continuously review the intake process and make recommendations for improvements in efficiency and client-centered practice.
Facilitator educational presentations about the organization and on brain injury.
Develop and distribute educational materials.
Attend community events to work at a promotional booth, and provide information about BCC services.
Familiarity with Google Suite and Microsoft Office Suite.
Provide reception coverage on a rotating basis.
Use databases to manage client information.
Provide assistance with advocacy and community referrals as required.
Continually investigate community resources to better serve individuals with acquired brain injury and maintain/update the resource database.
Contribute data and information for regular reports.
Pay: $46,000.00-$50,000.00 per year
