Job Summary:

The Access Coordinator is responsible for providing leadership to the admissions team, supporting centralized intake processes for all bed-based substance use programs. This role oversees the Program Navigators who support intake, transition, and discharge planning across all bed-based programs.

Responsibilities:
  • Coordinate the work of the Intake Specialists and Program Navigators on a day-to-day basis.
  • Ensure intake staff follow established criteria and prioritize intakes based on criteria, lead, and facilitate actions related to daily client flow, plan for client transfer or discharge, and appropriate client placement/referral.
  • Liaise with the clinical care team to ensure specific care plan actions and processes are completed and review the intake and discharge planning of clients to ensure optimal progression to discharge, in collaboration with other clinical team members.
  • Responsible for ensuring admissions team files and manages all verification paperwork as required to adhere to accreditation, licensing, and safety guidelines, and/or funding sources.
  • Act as a point person for clients, families, and community care teams who have concerns or questions regarding their care; follow up as required.
Administration:
  • Provide oversight related to clinical and organizational data collection across all MHSU programs; prepare reports for MHSU leadership and funders, as required.
  • Manage central client calendar to ensure program participants are supported by program navigators to attend scheduled appointments offsite as part of their treatment.
Human Resources:
  • Assist the Manager by attending panel interviews, providing recommendations on the selection of new staff, providing feedback to the Manager on staff performance appraisals, and optimal use of budgetary resources.
  • Determine related training and orientation requirements.
  • Assign and adjust workload, schedules, and coordinating staff when coverage is needed, ensuring service levels, and staffing requirements are met.
Community Engagement:
  • Participate in funder meetings and community stakeholder committees on behalf of the program as required by the MHSU leadership.
Qualifications:
  • Post-secondary diploma in social work, community services, etc., or an equivalent combination of education and experience.
  • 2 to 4 years of practical experience in an admission setting working with vulnerable populations, substance use services, mental health, or corrections.
  • Satisfactory completion of a criminal records check with vulnerable sector screening is required.
  • Satisfactory completion of Tuberculosis Screening form is required.
  • Satisfactory completion of Employee Immunization Record form is required.
  • Standard First Aid - with CPR-C-AED is required.
  • Valid driver's license with driver's abstract.
  • High level of integrity and dependability with a strong sense of urgency and results-orientation.
  • Demonstrated strong written, verbal, and interpersonal communication skills.
  • Ability to lead and work in a trauma-informed practice.
  • Demonstrated commitment to the social sector with a passion for the organization's mission is essential.

Salary:

The salary for this role is between $32.49 – $34.18 per hour.
