About the Role

We are seeking a highly skilled Care Coordinator to lead our mental health programs. As a key member of our team, you will be responsible for coordinating the work of intake specialists and program navigators, ensuring seamless client flow and optimal progression to discharge.

Responsibilities
  • Program Management:
  • Coordinate the daily activities of intake specialists and program navigators, prioritizing intakes based on established criteria.
  • Liaise with clinical care teams to ensure specific care plan actions are completed and review intake and discharge planning to optimize client progress.
  • Ensure admissions team files are up-to-date and manage verification paperwork as required.
  • Serve as a point of contact for clients, families, and community care teams, addressing concerns and following up as needed.
  • Administration:
  • Oversee clinical and organizational data collection across all MHSU programs, preparing reports for leadership and funders as required.
  • Manage the central client calendar, ensuring program participants attend scheduled appointments offsite as part of their treatment.
  • Human Resources:
  • Support the Manager by attending panel interviews, providing recommendations on new staff selections, and offering feedback on staff performance appraisals.
  • Determine related training and orientation requirements.
  • Assign and adjust workload, schedules, and coordinate staff coverage when needed, ensuring service levels and staffing requirements are met.
Qualifications
  • A post-secondary diploma in social work, community services, or an equivalent combination of education and experience.
  • 2-4 years of practical experience in an admission setting working with vulnerable populations, substance use services, mental health, or corrections.
  • Satisfactory completion of a criminal records check with vulnerable sector screening is required.
  • Satisfactory completion of Tuberculosis Screening form is required.
  • Satisfactory completion of Employee Immunization Record form is required.
  • Standard First Aid - with CPR-C-AED is required.
  • A valid driver's license with a driver's abstract.
  • A high level of integrity and dependability, with a strong sense of urgency and results-orientation.
  • Demonstrated strong written, verbal, and interpersonal communication skills.
  • Ability to lead and work in a trauma-informed practice.
Salary and Benefits

The estimated salary for this role is between $32,490 and $34,180 per year, depending on qualifications and experience.
