At Traverse Independence, we offer a unique opportunity to work with individuals who have acquired brain injuries (ABI) and support them in living independently in the community.

As an Outreach Worker, you will work in a low-barrier program, building trusted relationships with your clients to support their independence in the community. This is a rewarding role that requires strong interpersonal and organizational skills, as well as the ability to work effectively as part of a multidisciplinary team.

Key Responsibilities:

• Provide support to individuals in their homes or in community settings across the County of Wellington, including Rural Wellington.

• Work collaboratively with a team of regulated staff, including ABI specialists, an Occupational Therapist, Behaviour Therapist, and an Intensive Case Coordinator.

• Utilize your strong communication and interpersonal skills to build relationships with clients and support their independence goals.

Requirements:

• Education: Community College Diploma in Social Services, Developmental Services, Health Care, or related discipline.

• Demonstrated experience in behaviour management skills is a benefit, but not necessary as we offer a comprehensive training program.

• Valid driver's license and access to reliable vehicle with proof of $1,000,000 liability insurance and driver's abstract acceptable to Traverse Independence.
