Community Facilitator

**Traverse Independence** offers an incredible variety of supports and services for those who have an acquired brain injury (ABI) to live independently in the community. One of these exceptional services is our outreach program. As an outreach worker you will work in a low barrier program, building trusted relationships with your clients, which in turn will allow you to support them to work on independence in the community. This inspiring program makes a major difference in the lives of our clients and in partnership with your clients, you will experience great satisfaction as they achieve their goals, improve their quality of life and cope with their day-to-day activities.
You will report to an Outreach Coordinator, in a team that includes regulated staff such as ABI specialized psychiatrists, an Occupational Therapist, Behaviour Therapist and an Intensive Case Coordinator. You will provide support to individuals in their homes or in community settings across the County of Wellington, including Rural Wellington.
Experience with people with Acquired Brain Injuries is a benefit, but not necessary as we offer a comprehensive training program both internally and through the Ontario Brain Injury Association. This is a great opportunity to add an ABI specialization to your skillset.
**Community Facilitator Requirements**:
**Education**:
- Community College Diploma in Social Services, Developmental Services, Health Care, or related discipline.
Knowledge/Skills:
- Demonstrated experience in behaviour management skills is of benefit.
- Experience with people with disabilities such as mental health and/or addictions is a great asset but not necessary.
- Strong utilization and understanding of M365 - Teams/SharePoint.
- Demonstrated organizational skills, interpersonal skills, and communication skills.
- Ability to work as a member of a multidisciplinary team.
- Must have valid driver's license and access to reliable vehicle with proof of $1,000,000 liability insurance and driver's abstract acceptable to Traverse Independence
