About the Role

The University Health Network is seeking a skilled Social Worker to join our team at the Toronto Rehabilitation Institute - Lyndhurst Centre. As a Social Worker - Patient Care Specialist, you will play a vital role in providing exceptional patient care and support to individuals with complex needs.

Key Responsibilities

  • Assess patients' needs and develop personalized care plans to promote optimal health and well-being
  • Collaborate with interdisciplinary teams to provide comprehensive care and support
  • Facilitate family and team meetings to ensure effective communication and coordination
  • Reassess and modify care plans as needed to ensure optimal patient outcomes
  • Liaise with community agencies and healthcare providers to ensure seamless care transitions
  • Provide clinical teaching and supervision to students and colleagues
  • Participate in research and quality improvement initiatives to advance patient care and social work practice

Requirements

  • Master's degree in Social Work (or equivalent) from an accredited institution
  • Registration with the Ontario College of Social Workers and Social Service Workers
  • Membership in the Ontario Association of Social Workers (optional)
  • 2-3 years of experience working in a healthcare setting, preferably with an interprofessional team
  • Excellent communication, interpersonal, and problem-solving skills
  • Computer literacy and proficiency in relevant software applications
