Job Title: Clinical Social Worker
Job Summary:
Sunnybrook Health Sciences Centre is seeking a highly skilled Clinical Social Worker to join our team. As a Clinical Social Worker, you will provide a full range of social work services to patients and their families admitted to our critical care units.
Key Responsibilities:
- Provide psycho-social assessments, counseling, and end-of-life care to patients and their families
- Develop and implement advance care plans and goals of care conversations
- Link patients and families to community resources and support services
- Collaborate with interdisciplinary teams to provide comprehensive patient care
- Support patients and families in completing trauma psychosocial assessments
- Participate in quality improvement initiatives and contribute to the development of social work policies and procedures
Qualifications:
- Masters of Social Work degree required
- Member of the Ontario College of Social Workers and Social Service Workers in good standing
- Minimum two years of experience working in a hospital setting
- Experience in critical care preferred
- Exceptional communication and interpersonal skills
- Knowledge of the HCCA and SDM Act
- Knowledge of complex care management and complex health system access
- Strong negotiation and conflict resolution skills
- Demonstrated skills in inter-professional collaborative practice and education
- Ability to balance collaborative practice with self-direction
- Ability to manage multiple tasks and set priorities in a fast-paced environment
- Commitment to student education
- Experience and commitment to quality improvement initiatives
