About the Role

Spectrum Health Care is seeking a skilled Community Care Specialist to join our team. As a Patient Advocate, you will play a vital role in providing high-quality care to our clients in the Toronto area.

Key Responsibilities

  • Conduct comprehensive assessments of clients' needs and challenges, including physical, emotional, psychological, and social factors.
  • Develop tailored care plans to address clients' specific needs and goals, using a client-centered approach.
  • Collaborate with a multidisciplinary team to ensure a holistic and coordinated approach to care.
  • Provide ongoing support to clients, offering guidance and encouragement to help them navigate challenges and achieve their goals.
  • Identify and refer clients to appropriate community resources to help them reach their target goals.
  • Conduct regular follow-up visits to assess clients' progress and adjust care plans as needed.

Requirements

  • Completion of a Bachelor of Social Work from an accredited university (Master of Social Work an asset).
  • Registration with the Ontario College of Social Workers & Social Service Workers.
  • Minimum 5 years of experience working as a Social Worker.
  • Experience working in a community setting is an asset.
  • Valid Ontario driver's license and a reliable vehicle.
  • Proficiency in conducting assessments, interpreting findings, and formulating evidence-based treatment plans.
  • Familiarity with community resources and programs throughout the Toronto Central geography.
  • Proficiency in computer software programs (e.g., Microsoft Office) and virtual communication platforms (e.g., Microsoft Teams, Zoom, Go To Meeting).
