Position Overview

This role is dedicated to delivering clinical social work services to patients and their families within a hospital environment, ensuring exceptional patient care as part of a collaborative interdisciplinary team. Key responsibilities encompass: thorough assessment and treatment planning, educational and supportive counseling, resource identification, referrals, and collaboration with both inter-professional teams and community service providers.

Educational Requirements

Must be a member of the Ontario College of Social Workers & Social Service Workers or in the process of certification.
- A Master of Social Work from an accredited institution or an equivalent degree recognized by the Ontario College of Social Workers & Social Service Workers is essential.

Required Qualifications

- Recent experience in Maternal, Newborn, and Pediatric settings is advantageous.
- Familiarity with the Children’s Aid Society (CAS) is preferred.
- Experience in engaging with community resources is necessary.
- Ability to foster and maintain positive relationships with social work colleagues and professionals from other disciplines.
- Strong focus on service quality and innovation.
- Proven skills in comprehensive assessment and treatment planning.
- Demonstrated capacity for educational and supportive counseling.
- Ability to identify resources, make referrals, and facilitate connections.
- Experience in collaboration and consultation with inter-professional teams and community-based service providers.
- Capability to educate various groups, including social work professionals, students, patients, families, and community agencies.
- Involvement in professional, program, and hospital development, including planning, implementation, and evaluation of services.
- Excellent written and verbal communication skills are required.
- Experience in Maternal/Newborn/Pediatrics is preferred.
- Ability to prioritize and work efficiently in a dynamic environment.
- Commitment to a family-centered approach to care.
- Flexibility and adaptability to change are essential.
- A strong attendance record is required.
