Position Overview
Social Worker, Reactivation Care Centre
In this role, you will conduct comprehensive psychosocial evaluations to assess the emotional, physical, cognitive, and social requirements of patients and their families within the Reactivation Care Centre. Your expertise will be essential in assisting patients and families in developing coping strategies for the social and emotional challenges that may arise due to illness or hospitalization. You will leverage your extensive network within the healthcare community to connect patients with various community resources, including rehabilitation facilities and long-term care options.
Key Responsibilities
- Conduct thorough client-centered assessments to identify emotional, physical, cognitive, and social needs of patients and their families.
- Collaborate with the multidisciplinary team to create tailored treatment plans based on psychosocial assessments.
- Develop and implement discharge plans in conjunction with patients, families, and healthcare teams, considering the evolving needs and strengths of patients.
- Ensure seamless coordination of discharge plans and follow-up care.
- Facilitate comprehensive discharge planning, utilizing appropriate community resources.
- Provide guidance and support in adjustment and grief counseling.
- Maintain up-to-date knowledge of hospital and community resources, including access to social assistance and disability programs.
- Advocate for patients and families to ensure their rights are upheld and that they receive appropriate services.
- Promote an environment that fosters patient-centered care and inter-professional collaboration.
- Offer consultative social work services to treatment teams, hospital staff, and community partners.
- Participate in the supervision of MSW students.
Qualifications
- A current Police Vulnerable Sector Check is required.
- A Master's Degree in Social Work is mandatory.
- Registration with the Ontario College of Social Workers & Social Service Workers is essential.
- A minimum of one year of experience as a Social Worker in an acute care setting is preferred.
- Experience in geriatric care is an asset.
- Strong clinical skills with a demonstrated understanding of current professional practices.
- Proficiency in computer applications, including MS Word and patient information systems.
