Job Title: Social Worker - Mental Health Inpatient

Job Summary:

We are seeking a highly skilled Social Worker to join our Mental Health Inpatient team at North York General Hospital. As a Social Worker, you will provide patient-centered care to individuals and families, conducting biopsychosocial assessments, developing care plans, and connecting patients to community resources.

Key Responsibilities:

  • Conduct comprehensive patient-centered psychosocial assessments to identify emotional, physical, cognitive, and social needs, risks, and strengths.
  • Develop and implement treatment plans in collaboration with the multidisciplinary team and community partners.
  • Formulate discharge plans in collaboration with patients, families, and the healthcare team.
  • Guide patients in developing crisis plans to support their wellness and recovery after discharge.
  • Collaborate effectively as a member of the healthcare team and take initiative and leadership when appropriate.
  • Maintain excellent knowledge of and close liaison with hospital and community resources.
  • Advocate for patients' rights and ensure appropriate services are provided.

Requirements:

  • Master's Degree in Social Work.
  • Registration with the Ontario College of Social Workers & Social Service Workers (OCSWSSW).
  • Minimum of one-year experience as a staff Social Worker in an acute care setting.
  • Geriatric experience preferred.
  • Excellent clinical skills and proficiency with computers, MS Word, and patient information systems.
