Job Summary:
We are seeking a highly skilled Social Worker to join our Mental Health Inpatient team at North York General Hospital. As a Social Worker, you will provide patient-centered care to individuals and families, conducting biopsychosocial assessments, developing care plans, and connecting patients to community resources.
Key Responsibilities:
- Conduct comprehensive patient-centered psychosocial assessments to identify emotional, physical, cognitive, and social needs, risks, and strengths.
- Develop and implement treatment plans in collaboration with the multidisciplinary team and community partners.
- Formulate discharge plans in collaboration with patients, families, and the healthcare team.
- Guide patients in developing crisis plans to support their wellness and recovery after discharge.
- Collaborate effectively as a member of the healthcare team and take initiative and leadership when appropriate.
- Maintain excellent knowledge of and close liaison with hospital and community resources.
- Advocate for patients' rights and ensure appropriate services are provided.
Requirements:
- Master's Degree in Social Work.
- Registration with the Ontario College of Social Workers & Social Service Workers (OCSWSSW).
- Minimum of one-year experience as a staff Social Worker in an acute care setting.
- Geriatric experience preferred.
- Excellent clinical skills and proficiency with computers, MS Word, and patient information systems.

This is a Part-Time Regular position with 8-hour shifts, day shifts, in the Mental Health Unit.
