Job Summary

We are seeking a skilled Geriatric Nurse Specialist to join our team at Chartwell. This is a fantastic opportunity for a registered nurse with experience in geriatric, rehabilitation, or long-term/retirement nursing to make a meaningful impact in the lives of our residents.

About the Role

The successful candidate will be responsible for providing high-quality, patient-centered care to our residents, ensuring their physical and emotional needs are met. They will work closely with the multidisciplinary team to assess, plan, implement, and evaluate individualized care plans.

Key Responsibilities
  • Assess and prioritize the needs of residents, developing and implementing effective care plans in collaboration with the healthcare team.
  • Communicate effectively with residents, families, and other healthcare professionals to ensure seamless care coordination.
  • Participate in discharge planning, consultation, and/or transfer of residents as required.
  • Provide direct resident care, including administering medications, wound care, and vital sign checks.
  • Liaise with attending physicians, psychiatrists, consultants, social workers, clergy, relatives, or responsible persons to ensure effective resident care.
  • Procure and administer medications, including all injections and narcotics, in accordance with established policies and procedures.
Requirements

To be successful in this role, you must have:

  • A sound knowledge and background in geriatric, rehabilitation, or long-term/retirement nursing.
  • A current certificate of competence with the College of Nurses within your province.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
Compensation and Benefits

We offer a competitive salary and comprehensive benefits package, including health, dental, and vision coverage, as well as a generous retirement savings plan. The estimated annual salary range for this position is $80,000-$110,000, depending on experience.
