Job SummaryWe are seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will be responsible for assessing and treating complex geriatric patients with chronic health conditions in an acute or community/outpatient care setting. Your primary goal will be to enable clients and their families to live confidently and safely at home and/or in the community.
Key Responsibilities- Client Assessments and Care Planning: Conduct comprehensive assessments of clients' needs and develop personalized care plans to ensure optimal health outcomes.
- Care Coordination: Collaborate with interprofessional teams to coordinate care and services for clients, including referrals to other healthcare providers and community resources.
- Client Education and Support: Provide education and support to clients and their families on health promotion, self-management, and care self-management principles.
- Communication and Collaboration: Communicate effectively with clients, families, and healthcare professionals to ensure seamless transitions of care and facilitate information sharing.
- Documentation and Data Management: Maintain accurate and up-to-date client records and statistical data as required by established policies and procedures.
- Demonstrated knowledge and skills in gerontology and adults living with complex frailty and chronic illnesses.
- Ability to complete initial and ongoing client assessments and provide nursing care through therapeutic interventions.
- Excellent communication and interpersonal skills.
- Able to integrate and evaluate pertinent data from multiple sources to problem-solve effectively.
- Knowledge of broad healthcare services, community resources, and agencies providing a continuum of care.
- Ability to promote client-focused care, including sensitivity to diverse cultures and preferences.
- Physical ability to perform the duties of the position.
The estimated annual salary for this position is $88,416 - $104,464, depending on experience. We also offer a competitive compensation package, including four weeks of vacation, comprehensive health benefits, and a pension plan.
