We are seeking a highly skilled Registered Nurse to join our team as a Geriatric Nursing Care Coordinator.

**About the Role:**

The ideal candidate will possess a sound knowledge and background in geriatric, rehabilitation or long-term/retirement nursing, including activation, nursing, and personnel administration. Knowledge of relevant Federal, Provincial, and Municipal Statutes is essential for this role.

**Responsibilities:

  • Assess, plan, implement, and evaluate individualized care to meet residents' physical and emotional needs.
  • Communicate with relatives regarding resident care and condition.
  • Participate in discharge planning, consultation, and/or transfer of residents.
  • Provide direct resident care and participate in resident teaching.
  • Liaise with healthcare professionals to ensure effective resident care.

**Requirements:

  • A current certificate of competence with the College of Nurses within their province.
  • A strong understanding of geriatric nursing principles and practices.
  • Excellent communication and interpersonal skills.

**What We Offer:

We offer a competitive salary range of $85,000 - $115,000 per year, depending on experience, as well as opportunities for professional growth and development.
