Join Our Team as a Clinical Gerontological Nurse Specialist

We are seeking an experienced Registered Nurse/Registered Psychiatric Nurse to join our team as a Clinical Gerontological Nurse Specialist. This role will involve working collaboratively with the interdisciplinary team to provide person-centered care to diverse populations, promoting resident/client choices and individualized care plans.

About the Role:

  • Works collaboratively and in partnership with the interdisciplinary team to identify current or potential care concerns and strengths, plans interventions; documents and performs planned interventions and evaluates outcomes, as required.
  • Provides person-centered, direct nursing care (including wound care, dementia care and palliative care) and develops care priorities in accordance with established policies, procedures and standards of nursing practice.
  • Collects information from a variety of sources using skills of observation, communication and physical assessment to enable the provision of quality care.

Key Responsibilities:

  • Communicates resident/client assessments and changes to Physicians, Resident/Client Care Coordinator, and the interdisciplinary team.
  • Participates in care planning supporting decision making and person-centered care.
  • Acts as an advocate to protect and promote residents'/client's rights to autonomy, respect, privacy, dignity, diversity and access to information in accordance with standards of professional practice and philosophy of Residential Care/Acquired Brain Injury Services.

Requirements:

  • Graduate of an approved School of Nursing or Psychiatric Nursing.
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • One (1) year's recent, related gerontological nursing experience or an equivalent combination of education, training and experience.
