Salary and Benefits

Job Description

This role requires a registered nurse with current practicing registration with the British Columbia College of Nurses and Midwives. You will have at least one year of recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient care setting. Your expertise will be utilized to provide high-quality patient care, while collaborating with interprofessional teams and advocating for client-focused care.

Key Responsibilities
  • Establish therapeutic relationships with patients through interpersonal and interviewing techniques.
  • Assess clients' needs and develop individualized care plans in collaboration with healthcare professionals.
  • Provide direct client care, including interventions, referrals, and coaching to promote self-management and wellness.
  • Collaborate with primary care providers, families, and other healthcare professionals to ensure seamless transitions across care settings.
Requirements
  • Demonstrated knowledge and skills in gerontology and adult health care, particularly with complex frailty and chronic illnesses.
  • Proven ability to communicate effectively, both verbally and in writing.
  • Ability to integrate and evaluate pertinent data from multiple sources to problem-solve effectively.
  • Knowledge of broad health care services, community resources, and their roles in providing a continuum of care.
  • Physical ability to perform duties of the position.
