Vision Loss Rehabilitation Canada is seeking a skilled Client Care Navigator to join our team. This is an exciting opportunity to make a meaningful difference in the lives of our clients.

About the Role

The Client Care Navigator will play a crucial role in coordinating and integrating supports and services around individuals with vision loss. This includes assessing client needs, developing personalized care plans, and strengthening communication links between VLR staff and external healthcare providers.

Key Responsibilities:

  • Schedule appointments with primary and specialized service providers
  • Monitor client satisfaction and take remedial action as required
  • Provide proactive follow-up to ensure ongoing collaboration across services/providers
  • Identify opportunities for new or emerging care options to meet changing needs
  • Educate community partners about Vision Loss Rehabilitation Canada's services
  • Participate in multi-disciplinary team meetings
  • Foster good working relationships with professionals in other agencies and community groups

Requirements

To be successful in this role, you will require:

  • A diploma in Social Services Work, Community Work, or Mental Health
  • Experience working with individuals with vision loss
  • Excellent communication and interpersonal skills
  • Ability to prioritize workload and manage competing tasks
  • Proficiency in technology, including Microsoft Office and case management systems
