About the Opportunity

The Care Coordinator plays a vital role in assessing, planning, and authorizing services for clients referred to Continuing Care. This position requires strong assessment, interviewing, and negotiation skills to identify clients' goals, unmet needs, and resource requirements. The Care Coordinator works collaboratively with interprofessional teams to facilitate hospital discharge and return clients to their communities.

Responsibilities include:

  • Conducting comprehensive assessments to identify clients' needs and goals
  • Developing and implementing care plans to meet clients' needs
  • Authorizing services and making referrals to other resources
  • Working with clients and their care partners to identify and address unmet needs
  • Maintaining ongoing communication with clients, family, caregivers, and healthcare teams

The Care Coordinator is responsible for working in the community, hospital, or long-term care facilities, and must be fiscally responsible for the care, services, and equipment authorized within available Department of Seniors and Long Term Care policies and programs.

About You

We are seeking a highly skilled and compassionate individual with a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies. The ideal candidate will have a minimum of three years' experience in a community or long-term care setting and demonstrated knowledge and/or experience in dedicated case management or discharge planning.

The successful candidate will possess excellent assessment, interviewing, and negotiation skills, as well as excellent verbal and written communication skills. They will be able to work independently and collaboratively in a busy multidisciplinary situation, and have the ability to multi-task, prioritize workload, and manage time efficiently.

Hours and Compensation

This is a permanent full-time position with a 70-hour bi-weekly schedule. Shifts may include days, evenings, weekends, and holidays. The successful candidate will be compensated at a rate of $46.75 per hour and may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
