We are seeking a highly skilled and compassionate Community Health Advocate to join our team at Fraser Health Authority. As a key member of our interdisciplinary case management team, you will play a vital role in providing ongoing outreach services to individuals at risk or living with HIV who are difficult to engage or retain in care.

This is a Temporary Full-Time opportunity available until November 2025. We offer a comprehensive benefits package, generous vacation time, and a pension enrollment plan from day one.

The ideal candidate will have a Diploma in Community Social Services and two years' recent experience working with individuals living with HIV/AIDS, Hep C, mental health issues or an equivalent combination of education, training & experience. You will possess strong advocacy skills, the ability to work independently and in cooperation with others, and excellent communication and problem-solving skills.

As a Community Health Advocate, your primary responsibilities will include:

  • Participating in the development and implementation of care plans for assigned clients, identifying actual or potential problems and strengths, and working with the team to plan interventions.
  • Monitoring clients' progress, communicating and consulting regarding the care and support plan with the client and the case management team to ensure continuity of appropriate care for the client.
  • Creating linkages to other community resources and programs, contacting relevant organizations/businesses/services, and assisting clients with processing forms.
  • Communicating and consulting with the Nursing Supervisor and other team members about client care issues, participating in interdisciplinary conferences, and referring clients to community resources as appropriate.

You will also be responsible for maintaining accurate records of outreach activities, writing timely and accurate reports, and performing other related duties as assigned.

Based on the requirements of this position and the local market conditions, we estimate the salary range for this role to be CAD $31.56 - $33.28 per hour.
