Job SummaryWe are seeking a Community Health Nurse to join our team as a Clinical Care Coordinator. This role requires a skilled nurse to provide high-quality care to patients in their homes and communities. As a Clinical Care Coordinator, you will work collaboratively with interdisciplinary teams to assess, plan, implement, and evaluate patient care plans.
Responsibilities- Conduct comprehensive assessments of patients' physical and emotional needs, identifying areas for improvement and developing individualized care plans.
- Collaborate with healthcare professionals, family members, and caregivers to coordinate patient care, ensuring seamless transitions between services.
- Develop and maintain relationships with community resources, healthcare providers, and other stakeholders to promote patient well-being and access to necessary services.
- Provide education and support to patients and their families, empowering them to make informed decisions about their healthcare.
- Document patient progress, identify areas for improvement, and modify care plans as needed to ensure optimal outcomes.
- CURRENT PRACTICING REGISTRATION AS A REGISTERED NURSE WITH THE BRITISH COLUMBIA COLLEGE OF NURSES AND MIDWIVES (BCCNM)
- ONE YEAR RECENT RELATED CLINICAL EXPERIENCE ASSESSING AND TREATING COMPLEX GERIATRIC AND/OR ADULT PATIENTS WITH CHRONIC HEALTH CONDITIONS IN AN ACUTE OR COMMUNITY/OUTPATIENT CARE SETTING
- VALID BC DRIVER'S LICENSE AND ACCESS TO A PERSONAL VEHICLE FOR BUSINESS-RELATED PURPOSES
The salary range for this position is CAD $41.42 - $59.52 per hour.
