Job Summary

We are seeking a highly skilled and experienced Community Health Manager to lead our Neighborhood Health Team in Calgary, Alberta. As the new leader, you will collaborate with a regional team to oversee in-home care services, manage client programs, and oversee team operations.

About the Role
  • Direct and monitor the delivery of in-home services and daily functions of field staff, providing access to clinical advice, support, and resources.
  • Lead sales process and business development, client acquisition strategy, and marketing activities for your Neighborhood Health Team.
  • BUILD and sustain strategic relationships with current, new, and prospective partners and community organizations.
  • Assess and document community needs while developing business proposals to meet emerging opportunities.
  • Develop comprehensive plans for clients and families using a holistic approach to help them achieve aging in place goals.
Responsibilities
  1. Hire and train all NHT staff members, including training on services, products, and pricing.
  2. Supervise day-to-day performance of the team, ensure exceptional interactions with clients, and provide coaching and performance management.
  3. Assist with delivery of in-home services as required.
Requirements
  • CURRENT RN or LPN registration with College of Nurses of Alberta or registered Social Worker with Alberta College of Social Workers.
  • Minimum 5 years recent experience, preferably in a community setting, with at least 2 years managing others.
  • Valid CPR certification.
  • Experience in community work, working with community partners, business development, and engaging with municipal groups.
  • Reliable vehicle and willingness to travel to visit clients within the community.
Salary & Benefits

This is a full-time position with an estimated salary range of $80,000 - $110,000 annually, depending on experience. We offer a competitive benefits package, including health and dental coverage, retirement savings plan, and generous paid time off.
