Senior Community Health Care Manager

At Bayshore HealthCare, we are committed to delivering exceptional home care services that cater to the unique needs of our clients. As a Senior Community Health Care Manager with us, you will play a pivotal role in shaping the future of healthcare in Calgary, Alberta.
We are seeking an experienced and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to lead our team of healthcare professionals in providing high-quality care to clients in their homes. With a focus on client-centered care, you will work closely with our interdisciplinary team to develop comprehensive plans that address the diverse needs of our clients.
This is a hands-on community health position where collaboration with formal and informal care networks supports a client and family-centered home care model. You will be responsible for building relationships in the community, providing direct care in clients' homes, and supervising your team of healthcare professionals. As the new leader, you will collaborate with an integrated regional team to oversee a range of in-home care services, manage client service programs, and oversee all team operations.
The responsibilities of this role include:
- Developing comprehensive plans for clients and families using a holistic approach to help them achieve their aging in place goals.
- Directing and monitoring the delivery of all in-home services and daily functions of field staff, providing access to clinical advice, support, and resources.
- Hiring and training all healthcare staff and supervising the day-to-day performance of the team, providing ongoing coaching and performance management.
