  • As a Community Health Clinical Leader with Bayshore HealthCare Ltd., you will play a pivotal role in delivering exceptional home care services to clients in your local community.

About the Role:

">

This is a hands-on position that requires collaboration with formal and informal care networks to support a client and family-centered home care model.

">

You will build relationships in the community, provide direct care in clients' homes, and supervise a team of healthcare professionals. As the leader, you will collaborate with an integrated regional team to oversee a range of in-home care services, manage client service programs, and oversee all team operations.

Key Responsibilities:

  • ">
  • Develop comprehensive plans for clients and families using a holistic approach to help them achieve their aging in place goals.">
  • Direct and monitor the delivery of all in-home services and daily functions of field staff, providing access to clinical advice, support, and resources.">
  • Hire and train all healthcare staff and supervise the day-to-day performance of the team, providing ongoing coaching and performance management.">
  • Manage expectations with family members and clients, ensuring a client and family-centered approach to care planning.">
  • Adapt care to changing client needs, including occasionally working with the interdisciplinary team for management of acute illness, falls, and other immediate challenges.">
  • Ensure shift coverage for callouts and sick calls, providing direct hands-on care for clients if a replacement staff member is unavailable.

Requirements:

  • ">
  • A current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) registration with the College of Nurses of Alberta.">
  • Minimum of five (5) years of recent experience, preferably in a community setting, with at least two (2) years managing others.">
  • Experience in community healthcare and local community resources.">
  • Experience working with the geriatric population and their families is an asset.">
  • A reliable vehicle and willingness to travel to visit clients within the community, approximately 60% of the time.

Compensation:

The estimated salary for this position is $80,000 - $110,000 per year, based on the location and industry standards in Calgary, Alberta.
