About the Role

Nisa Foundation is seeking a highly skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a critical role in supporting women in need, providing them with the necessary resources and guidance to overcome challenges and achieve their goals.

Key Responsibilities
  • Case Management: Develop and implement individualized case plans to address the unique needs of each client, ensuring they receive comprehensive support and services.
  • Assessment and Intervention: Conduct thorough assessments to identify client needs, and provide effective interventions to address these needs, promoting positive outcomes and empowering clients to take control of their lives.
  • Communication and Collaboration: Build and maintain strong relationships with clients, colleagues, and community partners, ensuring seamless communication and collaboration to achieve shared goals.
  • Crisis Intervention: Develop and implement crisis intervention strategies to address high-risk situations, ensuring the safety and well-being of clients and staff.
  • Documentation and Reporting: Maintain accurate and detailed records of client interactions, progress, and outcomes, providing regular reports to stakeholders as required.
Requirements
  • Education: Degree or Diploma in a related human/social service field, such as Social Work or Social Service Work, or Child and Youth Care Work, with a minimum of one year of recent related experience.
  • Skills and Knowledge: Proven skills in case management, assessment, and intervention, as well as crisis-intervention methodologies, conflict resolution, and negotiation.
  • Personal Qualities: Demonstrated passion for supporting women, resilience, positive attitude, reliability, emotional intelligence, and strong critical thinking skills.
