Job Summary

The Primary Health Nurse II provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team, the Primary Health Nurse II provides care to individuals promoting wellness, prevention of disease and/or disability and the management of chronic diseases/conditions.

The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals.

Key Responsibilities
  • Provide health education and care to individuals within the scope of nursing practice
  • Work as an advocate on behalf of the client to ensure appropriate links/referrals to other human services
  • Collaborate with interdisciplinary teams to provide client-centered care
  • Develop and implement care plans to promote wellness and prevent disease
  • Provide education and support to clients and their families on health-related topics
Requirements
  • Graduate of approved Registered Nursing education program, BN preferred
  • Member in good standing with CRNM
  • A minimum of five (5) years' experience working as a Registered Nurse
  • A minimum of three (3) years' experience working in a community health setting
  • Knowledge and Experience working with Students and/or Volunteers
  • Current CPR Certification at Basic Rescuer Level
Preferred Qualifications
  • Proven knowledge and ability in critical thinking, decision-making, problem-solving, and crisis management
  • Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes
  • Commitment to the principles that guide Mount Carmel Clinic, as reflected in the history, vision, mission and values
  • Knowledge and understanding of Indigenous culture and traditions and the impact of colonization on the health and well-being of Indigenous individuals and families
  • Proven ability to work within an interdisciplinary team environment with respect, professionalism, cooperation, sensitivity and cultural awareness
