About the Role

As a Clinical Care Coordinator with SE Health, you will play a vital role in delivering high-quality care to clients in their homes and communities. This is an exciting opportunity to work in a dynamic environment where you can make a meaningful difference in the lives of others.

This part-time role offers a unique schedule, working Thursday and Friday one week and Saturday and Sunday the next. You will be part of a diverse team that provides comprehensive care to clients across various settings.

Responsibilities
  • Provide continuous and comprehensive support to your team in the delivery of care for your clients
  • Complete client assessments and develop interdisciplinary care plans based on best evidence and practices
  • Lead the care planning process and work with other disciplines to implement client care plans
  • Facilitate regular interdisciplinary team communication and lead discharge and transition care planning discussions
  • Support clients, families, and caregivers through care transitions by collaborating and connecting them with community support services
Requirements

To be successful in this role, you must possess:

  • A degree in nursing (BSc or BSN)
  • Registration with the College of Nurses of Ontario
  • A minimum of two years of experience in nursing, preferably in home and community care
  • Demonstrated clinical leadership experience and strong assessment skills
  • Excellent interpersonal, communication, and critical thinking skills
