We are seeking a highly skilled and experienced Senior Clinical Team Lead to join our team at ParaMed Home Health Care. This is an exciting opportunity to lead a team of healthcare professionals in providing exceptional care to our clients in their homes.

Job Summary

The Senior Clinical Team Lead will be responsible for supervising and supporting a team of Registered Nurses (RNs) and Registered Practical Nurses (RPNs) to ensure that our clients receive high-quality, patient-centered care. The successful candidate will have excellent leadership and communication skills, as well as a strong understanding of community nursing practice.

Key Responsibilities

  • Supervise and support RNs and RPNs to ensure compliance with legislated standards of practice, company policies, and procedures.
  • Respond to and advise on concerns from community staff, patients/families, and other team members.
  • Provide ongoing leadership through regular huddles, face-to-face meetings, and one-on-ones.
  • Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.
  • Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
  • Engage patients in problem-solving using a collaborative approach, offering health teaching, building rapport, and trust to identify patient health care needs, strengths, capacities, and goals.
  • Engage staff in problem-solving, offering education, coaching, and mentoring in resolution of problems.
  • Maintain knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice settings that affect scope of practice.

Requirements

  • Current RN or RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions.
  • Minimum of 5 years' experience as a practicing Registered Nurse or Registered Practical Nurse.
  • Comprehensive patient health assessment skills required.
  • Possess an understanding of the principles of community nursing practice.
  • Proven experience leading and managing a team.
  • Knowledge of clinical documentation standards and practice guidelines required.
  • Excellent organizational, interpersonal, and teaching skills.
  • Commitment to professional development in clinical practice relevant to home health care.
  • Effective critical thinking and problem-solving skills.
  • Must possess a valid driver's license and access to reliable transportation.
  • Able to provide a current, clear Vulnerable Sector Screening (VSS).
  • C.N.A. Certification as a Community Health Nurse (CCHN(C)) considered an asset.

Salary and Benefits

The salary for this position is $1500 per week.
