Clinical Care Specialist

Under the direction of the resident services manager, the successful candidate will provide care to assigned residents/tenants and guide attendants in their duties. The individual must adhere to all regulations and standards in accordance with the Retirement Homes Regulatory Authority.
Background/Qualifications
A registered practical nurse with a current Ontario Certificate of Competence from the College of Nurses of Ontario is required. The candidate must possess a medication administration certificate and be able to follow oral and written instructions in the English language.
Key Responsibilities
The successful candidate will participate in ChelseyPark's Quality Improvement Program and contribute to the development of resident care plans. They will assess the needs of individual residents/tenants and organize care to promote maximum independence in conjunction with the resident services manager.
The candidate will carry out planned assignments and job routines for effective resident and tenant care, taking into account physical, spiritual, social, and emotional needs. They will supervise the nursing care given by attendants and unregulated care providers or designated staff, in accordance with resident care plans.
The registered practical nurse will provide direct resident/tenant care, adhere to current and approved standards of care, policies, and procedures. They will assist with the implementation and enforcement of all ChelseyPark's policies.
The candidate will maintain effective communication and good interpersonal relations with medical and allied staff, residents, tenants, families, and other departments. They will provide emergency medical assistance and summon appropriate intervention and notify families of the same.
Accurate records of nursing observations and procedures performed will be maintained. The candidate will assist with safe keeping of all drugs and nursing solutions in the residence. They will participate in the development and implementation of a plan of care for residents that supports independence as a member of the multi-disciplinary team.
The successful candidate will support and actively participate in unit orientation and complete applicable educational training. They will assist the resident services manager in evaluating the performance of attendants and unregulated care providers and assist with appropriate documentation. The candidate will keep the resident services manager informed of any staff members whose performance is less than satisfactory.
The registered practical nurse will administer medication in accordance with the College of Nurses standards as an added nursing skill and in compliance with the policies of ChelseyPark. They will consult with tenants and residents and their families and/or physicians regarding the need for varied service interventions and/or referrals.
The candidate will record all charges for services on individual resident/tenant sheets. They will comply with the Occupational Health and Safety Act, Workplace Safety and Insurance Act, and Workplace Hazardous Material Information System. This includes recognizing health and safety hazards, reporting incidents, fulfilling responsibilities under the applicable legislation, as well as participating in educational sessions and fire drills.
The successful candidate will perform other related duties as delegated by the Resident Services Manager.
