Clinical Care Leader

About the Role:
The Clinical Care Leader will be responsible for the supervision of Nurses who are providing care and making a difference for clients within the community.
Key Responsibilities:
- Supervise and support Nurses to ensure practices comply with all legislated standards of practice and company policies/procedures.
- Respond to and advise/resolve concerns from community staff, patients/families, and other team members.
- Provide ongoing leadership through regular meetings, face-to-face interactions, and one-on-ones.
- Initiate patient safety incident investigations and document to ensure timelines and action plans are met.
- Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
- Engage patients in problem-solving using a collaborative approach, offering health teaching, building rapport, and trust to identify patient health care needs, strengths, capacities, and goals.
- Engage staff in problem-solving, offering education, coaching, and mentoring in resolution of problems.
Requirements:
- Current RN registration with the College of Nurses of Ontario and entitled to practice without restrictions.
- Minimum of 5 years' experience as a practicing Registered Nurse.
- Comprehensive patient health assessment skills are required.
- Possess an understanding of the principles of community nursing practice.
- Proven experience leading and managing a team.
- Knowledge of clinical documentation standards and practice guidelines is required.
- Excellent organizational, interpersonal, and teaching skills.
- Commitment to professional development in clinical practice that is relevant to home health care.
- Effective critical thinking and problem-solving skills.
- Must possess a valid driver's license and access to reliable transportation.
- Able to provide a current, clear Vulnerable Sector Screening (VSS).
- C.N.A. Certification as a Community Health Nurse (CCHN(C)) is considered an asset.
