About the Role:

The Clinical Supervisor will oversee the supervision of PSWs and HSWs, ensuring they provide high-quality care to clients in the community. This role requires expertise in assessing, implementing, and evaluating clinical care interventions to guarantee safe, quality, and ethical care.

Key Responsibilities:

  • Supervise and support PSWs and HSWs to ensure compliance with legislated standards and company policies.
  • Respond to concerns from community staff, patients, families, and team members.
  • Provide ongoing leadership through regular meetings, huddles, and one-on-ones.
  • Investigate patient safety incidents, documenting timelines and action plans.
  • Collaborate with nursing team members to support clinical documentation and submission requirements.
  • Engage patients in problem-solving, offering health teaching, building rapport, and identifying patient healthcare needs.
  • Engage staff in problem-solving, offering education, coaching, and mentoring.
  • Maintain knowledge and skills to provide safe, efficient, and effective service.

Requirements:

  • Current RN or RPN registration with the College of Nurses of Ontario.
  • Minimum of 5 years' experience as a practicing Registered Nurse or Registered Practical Nurse.
  • Comprehensive patient health assessment skills.
  • Understanding of community nursing practice principles.
  • Proven experience leading and managing a team.
  • Knowledge of clinical documentation standards and practice guidelines.
  • Excellent organizational, interpersonal, and teaching skills.
  • Commitment to professional development in clinical practice.
  • Effective critical thinking and problem-solving skills.
  • Valid driver's license and access to reliable transportation.
  • Able to provide a current, clear Vulnerable Sector Screening (VSS).
  • C.N.A. Certification as a Community Health Nurse (CCHN(C)) is considered an asset.
