Salary:
$30.48 per visit, with opportunities for additional compensation based on experience and performance.
About the Role:
We are seeking a Registered Practical Nurse (RPN) to join our team as a Clinical Nurse Specialist in Home Care. In this role, you will be responsible for providing skilled clinical care to clients in their homes, working independently and as part of a multidisciplinary team to deliver high-quality care.
Key Responsibilities:
- Providing direct patient care, including assessments, wound care, and medication management
- Developing and implementing individualized care plans
- Maintaining accurate records and documentation
- Collaborating with other healthcare professionals to ensure seamless care transitions
- Participating in education and training programs to enhance skills and knowledge
Required Qualifications:
- Current registration as a Registered Practical Nurse with the College of Nurses of Ontario
- Demonstrated experience in home care or a related field
- Strong communication and interpersonal skills
- Able to work independently and as part of a team
- Valid driver's license and reliable vehicle
