Job Summary:

Sunrise Senior Living is seeking a skilled and compassionate Nurse - Resident Care Specialist to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the overall well-being of our residents.

Responsibilities:

  • Conduct regular wellness visits for all residents to identify on-going needs and services.
  • Collaborate with the Resident Care Director to evaluate and document resident care plans.
  • Monitor and follow up with residents who have experienced falls, accidents, or incidents.
  • Document all pertinent information in resident wellness files.
  • Communicate with residents' families and responsible parties to ensure medical needs are met.
  • Coordinate with attending physicians as necessary.
  • Ensure weights and vital signs are obtained monthly for each resident.
  • Assist in maintaining wellness files according to Sunrise policies and federal regulations.
  • Provide clinical support and assistance to community team members as needed.
  • Understand and follow infection control practices.

Qualifications:

  • Graduate of an approved college or school of nursing.
  • Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
  • Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
  • Ability to handle multiple priorities and possess excellent communication and organizational skills.
