Job Title: Outreach Worker, Overdose Outreach Team

About the Role

Fraser Health is seeking an experienced Outreach Worker to join our Overdose Outreach Team. As a key member of our interdisciplinary team, you will provide support services to individuals and their families accessing opioid agonist therapy and related support services.

Key Responsibilities

  • Monitor clients' health conditions and environments, documenting behavioral, physical, and cognitive changes as needed.
  • Arrange and escort clients to medical appointments and errands, providing emotional and physical support.
  • Create linkages to community resources and programs, assisting clients with accessing benefits and services.
  • Administer medication to clients under the direction of a Registered Nurse and provide medication reminders.
  • Act as an advocate for clients with government agencies, healthcare providers, and landlords.
  • Identify potential emergency situations and make appropriate interventions.
  • Provide basic health-promotion information and promote development of life-skills in clients.
  • Perform general clerical duties, including maintaining client files and attending meetings.

Qualifications

  • Completion of Substance Use Certificate and one year's recent related experience working with clients with substance use issues.
  • Valid BC driver's license and vehicle.
  • Current CPR certificate.
