We are seeking an experienced Outreach Worker to join our Forensic Psychiatric Services team in Surrey, BC. As an Outreach Worker, you will play a critical role in providing support and services to individuals with complex mental health and substance use challenges.
Key Responsibilities- Conduct assessments and develop service plans to support clients in achieving their goals and improving their overall well-being.
- Provide education and support to clients on topics such as mental health, substance use, and harm reduction.
- Collaborate with other healthcare professionals to ensure comprehensive care and support for clients.
- Develop and maintain relationships with community resources and services to ensure clients have access to necessary support.
- Participate in team meetings and contribute to the development of policies and procedures.
- Graduation from a recognized Social Services Worker certificate program or equivalent combination of education and experience.
- Valid Class 5 vehicle license.
- Knowledge of Indigenous Cultural Safety and anti-racism principles and practices.
- Ability to communicate effectively and work collaboratively with diverse populations.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
