Job Title: Outreach Worker, Overdose Outreach Team

Fraser Health is seeking a highly motivated and compassionate Outreach Worker to join our Overdose Outreach Team. As a key member of our interdisciplinary team, you will provide critical support services to individuals and their families affected by substance use issues.

Key Responsibilities:
  • Provide outreach and support services to clients, including medication administration and reminders, under the direction of a Registered Nurse.
  • Assist clients in accessing benefits and assistance, and provide brief planning and coordination on issues related to substance use, opioid agonist therapy, overdose prevention, and harm reduction.
  • Monitor clients' health conditions and environments, documenting changes and reporting significant observations to the Registered Nurse or delegate.
  • Establish linkages to other community resources and programs, and provide advocacy as needed.
  • Perform a variety of general clerical duties, including responding to inquiries, maintaining client files, and utilizing computer software applications.
Requirements:
  • Completion of a Substance Use Certificate, plus one year's recent related experience working with clients with substance use issues.
  • Valid BC driver's license and vehicle.
  • Current CPR certificate.
  • Demonstrated ability to work with marginalized and culturally diverse populations, including those with substance use issues and mental illness.
  • Excellent communication and interpersonal skills.
