Outreach Worker, Overdose Outreach Team

Job Summary:
We are seeking a highly motivated and compassionate Outreach Worker to join our Overdose Outreach Team. As a key member of our interdisciplinary team, you will provide support services to individuals and their families, helping them access opioid agonist therapy and related support services.
Key Responsibilities:
- Monitor clients' health conditions and environments, documenting any changes or concerns.
- Arrange and accompany clients on appointments, errands, and other activities, providing emotional and physical support.
- Create linkages to community resources and programs, assisting clients with accessing benefits and services.
- Administer medication to clients under the direction of a Registered Nurse, providing medication reminders as needed.
- Act as a liaison between agencies, providing information on services and updating team staff on available resources.
- Identify client needs and make referrals or develop strategies for support, education, and follow-up care.
- Provide basic health-promotion information, promoting development of life-skills and client empowerment.
- Perform general clerical duties, attending meetings and participating in patient care planning and coordination.
Qualifications:
- Completion of Substance Use Certificate, plus one year's recent experience working with clients with substance use issues.
- Valid BC driver's license and vehicle.
- Current CPR certificate.
Skills and Abilities:
- Demonstrated ability to work with marginalized and culturally diverse populations.
- Excellent listening and communication skills.
- Ability to establish rapport with clients and families.
- Working knowledge of harm reduction principles.
- Crisis prevention/intervention and conflict resolution/de-escalation skills.
