Job Summary

We are seeking an experienced Outreach Worker to join our Forensic Psychiatric Services team. As an Outreach Worker, you will play a critical role in providing support and services to individuals with complex mental health and substance use challenges.

Key Responsibilities
  • Conduct assessments and develop service plans to address the unique needs of clients
  • Provide outreach and engagement services to clients in their communities
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support
  • Develop and maintain relationships with community partners and stakeholders
  • Participate in quality improvement initiatives to enhance service delivery
Requirements
  • Graduation from a recognized Social Services Worker certificate program or equivalent combination of education and experience
  • Valid Class 5 vehicle license
  • Knowledge of Indigenous Cultural Safety and anti-racism principles
  • Ability to communicate effectively and work collaboratively with diverse stakeholders
  • Physical ability to carry out the duties of the position
