Forensic Psychiatric Services Outreach Worker

We are seeking an experienced Outreach Worker to join our Forensic Psychiatric Services team. As an Outreach Worker, you will play a critical role in providing support and services to individuals with complex mental health and substance use challenges.
Key Responsibilities- Conduct assessments and develop service plans to address the unique needs of clients
- Provide outreach and engagement services to clients in their communities
- Collaborate with multidisciplinary teams to ensure comprehensive care and support
- Develop and maintain relationships with community partners and stakeholders
- Participate in quality improvement initiatives to enhance service delivery
- Graduation from a recognized Social Services Worker certificate program or equivalent combination of education and experience
- Valid Class 5 vehicle license
- Knowledge of Indigenous Cultural Safety and anti-racism principles
- Ability to communicate effectively and work collaboratively with diverse stakeholders
- Physical ability to carry out the duties of the position
