Forensic Psychiatric Services Outreach Worker

BC Mental Health & Substance Use Services is seeking a highly skilled and compassionate Outreach Worker to join our Forensic Psychiatric Services team in Surrey, BC.
Job Summary:We are looking for a dedicated and empathetic individual to provide outreach services to clients with complex mental health and substance use challenges. As an Outreach Worker, you will work collaboratively with our multidisciplinary team to provide support, advocacy, and connection to community resources.
Key Responsibilities:- Conduct assessments and develop individualized plans to support clients in achieving their goals.
- Provide emotional support, guidance, and education to clients and their families.
- Establish and maintain relationships with community agencies and organizations to access resources and services.
- Collaborate with healthcare providers and other professionals to ensure comprehensive care.
- Document client progress and outcomes in a timely and accurate manner.
- Graduation from a recognized Social Services Worker certificate program or equivalent combination of education and experience.
- Valid Class 5 vehicle license.
- Knowledge of Indigenous Cultural Safety and anti-racism principles.
- Ability to communicate effectively with diverse populations.
- Strong organizational and time management skills.
